Job Summary

Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Sacramento, California. In this hybrid role, you will conduct in-person preadmission evaluations and assessments throughout Sacramento.

As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. You will be responsible for completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), Related Conditions (RC), or geriatric populations across nursing homes, hospitals, and community-based settings. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.

This full-time Clinical Evaluator position offers both flexibility and purpose—combining remote work from your home office with local travel. Acentra Health covers and reimburses all travel-related expenses, including mileage and parking. Additionally, we provide all necessary technology and equipment for your home office setup, including a laptop for use while in the field.


Job Responsibilities

  • As a Clinical Evaluator, you will conduct PASRR Level II assessments and evaluations, including reviewing relevant records and gathering input from collateral sources.
  • Conduct a thorough and accurate PASRR Level II reviews by evaluating patients’ physical, behavioral, and psychological functioning to determine the most appropriate level of care.
  • Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations.
  • Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements per State and Federal Regulation
  • Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.
  • Provide well-informed, clinically sound Level of Care recommendations based on thorough evaluations.
  • Knowledge of standards of practice related to Level of Care determinations, as well as mental health, geriatrics, and other conditions commonly requiring long-term care placement.
  • Ensure compliance with established workflows, including re-evaluations of records based on psychiatric input.
  • Understand, and comply with all corporate policies, including those related to HIPAA and its Privacy and Security Rules.
Qualifications:


Required Qualifications, Knowledge, and Experience

  • Must possess an active, unrestricted license in the State of California in a healthcare or related field, such as Licensed Psychologist (LP), Licensed Professional Clinical Counselor (LPCC), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Professional (LMHP), Registered Nurse (RN) and Psychiatric Mental Health Nurse Practitioner (PMHNP).
  • Graduation from an accredited college with a relevant degree program and a minimum of 2 - 3 years of experience in assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.

  • Minimum 1 year of experience working with individuals with intellectual or developmental disabilities or related conditions.

  • Solid understanding of standards of practice related to Level of Care determinations and familiarity with standards pertaining to mental health, geriatrics, and other conditions that often require long-term care placement.

  • Demonstrated expertise in assessing and analyzing an individual’s psychological, emotional, behavioral, and physical functioning.
  • Experience with medical record abstraction is required
  • Effective communication skills with the ability to interact effectively with individuals and groups through verbal, written, and electronic formats.
  • Detail-oriented with strong organizational skills and able to prioritize and complete tasks efficiently within set timelines.
  • Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).

 

Preferred Qualifications

  • Knowledge of Federal Regulation 42 CFR, Chapter 483 related to long-term care and resident assessment requirements.

  • Understanding of psychiatric disorders as defined in the most current edition of the DSM , per applicable state guidelines.

  • Understanding of Preadmission Screening and Resident Review (PASRR) requirements in accordance with state and federal regulations. 
  • Knowledge of mental health, intellectual and developmental disabilities (IDD), geriatrics, and other conditions requiring long-term care services and supports.

  • Familiarity with clinical standards and Level of Care determination across healthcare settings.

  • Knowledge of public mental health systems, including public services, supports, and Level of Care guidelines.

  • Previous experience working with the geriatric population is preferred.

Compensation

The compensation for this role is $84,000.00 to $89,000.00 base salary

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
