Job Summary and Responsibilities: Acentra Health is looking for passionate Clinical Evaluators (PASRR) to join our growing team! Job Summary As a Clinical Evaluator on our Pre-Admission Screening and Resident Review (PASRR) team, you will play a vital role in ensuring individuals receive the right care at the right time. By conducting face-to-face assessments in Scottsbluff, Nebraska, and its surrounding areas. As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. . These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes. This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. All travel expenses—including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field.  Job Responsibilities Responsible for conducting PASRR (Preadmission Screening and Resident Review) assessments and evaluations, and for developing informed level-of-care recommendations across diverse settings—including nursing homes, hospitals, and community environments to effectively understand and address the unique needs of each individuals served. Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports. Ensure compliance with contractual and regulatory standards while developing level of care recommendations Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients. Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements. Ensure compliance with established workflows, including re-evaluation of records based on psychiatric input. Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics. Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities. Execute tasks with strong attention to detail, ensuring adherence to established workflows to maintain high standards of quality. Qualifications: Required Qualifications, Skills, Knowledge and Experience Must possess an active, unrestricted license in the State of Nebraska in one of the following: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), Registered Nurse (RN). Graduation from an accredited college or university with a degree relevant to the field, and minimum of 2 years of experience assessing and diagnosing individuals with mental or psychiatric disorders—including expertise in complex differential diagnoses involving substance use, medical conditions, cognitive impairments, and dementia. Minimum 1 year of experience working with individuals with intellectual disabilities or related conditions. Working knowledge of the Preadmission Screening and Resident Review (PASRR) requirements as per Federal Regulation and the state PASRR rules. Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning to develop appropriate level of care recommendations based on clinical evaluations. Familiarity with public mental health and medical programs. Strong verbal, written, and electronic communication skills with the ability to engage effectively with individuals and groups. Detail-oriented with strong organizational skills and the ability to prioritize and complete tasks efficiently within established timelines. Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams). Knowledge of psychiatric disorders, as outlined in the current Diagnostic and Statistical Manual (DSM) used in Nebraska, is preferred.
